Program Description:
TTC will provide transitional housing for chronically homeless adults with general needs from ages 18 and up.  The program will house 12 persons at any point in time.  TTC will provide 2 private leased homes, each home will have 3 bedrooms for a total of 12 beds.  Each participant will receive supportive services to include but not limited to intensive case management, individual substance abuse and crisis counseling along with mental health services.
