Program Description:
SFVMHC proposes to provide six months of transitional housing to 30 homeless individuals with multiple diagnoses.  There are 10 rooms dedicated to this program in the Valley shelter.  There will be 3 clients per room.  Supportive services include 24-hour staffing, intensive case management, drug abuse treatment, mental health services and counseling.
