Program Description:
Access center providing information, orientation, referrals and encouragement to all homeless sub-populations, assesses participants needs and then connect to appropriate services, connect participant to LAFHC emergency, transitional and permanent housing program and provide outreach services to the homeless community. Based on " ONE STOP" model of service delivery.
